Abstract

This study investigates the effect of deductibles in the Swiss social health insurance system on health care expenditure (HCE). We employ annual HCE data for the years 1997-1999 of about 75.000 persons in the canton of Zurich . The law sets a minimal statutory annual deductible of € 160 but allows individuals to opt for a higher deductible (270 €, 410 € 820 € or 1030 €). The task for identifying the effect of deductibles on HCE is challenging since individuals select contracts based on their expected future HCE. Taking into account the endogeneity of the contract choice we show that deductibles reduce HCE significantly.
